Reference Form AND Psychological Practice Profile
The Reference Form & the Psychological Practice Profile (which will first need to be completed by you) must be provided to your three referees. You can send this form to each referee by email, fax or regular mail. However, once this form is completed, each referee must send his/her signed copy of the form directly to the NSBEP by regular mail (though an initial copy may be sent by fax or email). The Board requires three references. Two of these referees must be registered or licensed psychologists.

The Profile must be completed and a copy provided to the each of the three individuals providing a reference for you.

Confirmation of Standing Form
If you are registered in any capacity with another psychology regulatory body, this form must be completed by the regulatory authority in the respective jurisdiction.
